Par Q or other screen?

Does anyone prescreen their clients with this Par Q form or similiar? WFS

Or do you just have them do a waiver and not concern yourself with the particulars?

Currently we are using just the waiver. However, I plan to add the ParQ as well beginning in January when we go to an online system.

We have always used the Par Q. By law, some states require you to ask certain questions. WA state requires it in the big boxes. To me, I want to know! Bone, joint, back, heart concerns? You bet, I want to know!

Upon completion of the document, if they have answered yes, I say,"Tell me about your bone or joint problem", or whatever the concern is. At times, depending on the condition, I may ask them to provide me with a physicians release to perform physical exercise before I continue, to cover my butt.
