I have my own mental list of errors but your input would be appreciated too.

You're weight is shifted forward from the start and it never gets back, mainly because your hips are too high.

Drop your hips and get your weight centered on your feet.

move your feet when you catch it. you'll be more balance and have a stronger base of support.

I agree that your weight is too far forward however, I disagree on the reason.
Move the video frame by frame and you will see that you raise your hips and extend your knees leaving your chest over the bar and your back flat. You then try to pull and drop to receive the weight. This pull sets you way off balance because you are already in a poor position. Then you push the bar forward altering the bar path. This extenuates the problem and you have to fight to keep the bar stable. Clearly your legs are stronger than your back and because of this you feel that your positioning is correct, but its the small things that are hampering your attempts as soon as you start them.

I hope to see more of your videos where you are completing higher weight!

1. Back isn't set from the start. It's not tight.

2. Are you releasing the hook grip on the catch?
