You had the bar too far in front of you. When you crossed the knee and started the sweep back, and begin your acceleration your heel came off the ground way before you hit extension, meaning your center of balance was too far forward: http://new.tinygrab.com/a1cb0332f971...3ae58fc002.png (wfs). This piggy backs on what Jeff said. Biggest fault is that you aren't quite patient enough and don't trust your technique to carry you through, so you try to muscle it and get over-eager which throws you off balance. You're plenty strong enough to stick this weight, just need to back the weight down and dial technique over and over until you feel confident in the groove (mental) and catch (execution).
