Joe, this sounds like a classic example of misalignment. I mean that your body is using compensating patterns outside of its design. We can get by for a while doing this, but it eventually manifests in pain or injuries like plantar fasciitis. In your case, something is affecting your gait, causing your heel to strike improperly.

This is why the pain went away when you ceased some activities and then came back--the underlying dysfunction wasn't fixed. This is also why you feel the pain spreading up your leg.

PLEASE contact me--I can help. I'm a CF-L1 and deal with teaching people how to fix these compensating patterns on their own. Also, orthotics are weakening the arch muscles of your feet and will make things worse down the road.
