it is hard to tell from that angle (have your camera man hold the camera at the same height as your knees) but you may have gotten enough depth on that first 160 lift, or at least it looked like it to me.

Either way form looked good for a max effort.

As for failing part way up, i actually had the same thing on my last rep of three sets of five reps. I think it is because you are able to get the stretch reflex to help drive out of the hole but then the muscles fatiuge and cannot get you the rest of the way up. It is not uncommon and I would not worry about it, just keep training squats and youll get past it
