It almost looks like a balance issue to me, upper body balance and lower body balance are very different was one outweighs the other.
Try not leaning forward and acting like you are sitting in a chair with your hands out just sink down with your butt over your feet. Point your toes out and don't be afraid to fall put your knees forward like your sitting on your heels while on your tip toes. Then slowly recover from that position till it becomes a breeze will work on your balance and form.
