You're clearly limited by the jerk. Dip went forward, sending the bar forward. Put a bend in that back leg in the landing position; land on the ball of the foot and don't let the foot point out.

Absolutely not. In fact setting the damper to "10" is usually a bad, bad idea, especially for beginners who haven't learned proper mechanics yet.

The bar needs to land on meat, not bone. Check your receiving position; if the bar doesn't have full contact all the way across the shoulders you're doing it wrong. No gaps!

Are they protracting the shoulders or retracting them? If they are taught to retract the shoulders they absolutely will not be able to rack the bar properly....

You get more ankle flexion in your OHS than I do. So I'd suggest that it's the hips that are actually the limiting factor and the ankles are merely trying to make up the difference.

I consider any of the IWF certified bars - Eleiko, DHS, Uesaka, Werksan, Zhangkong - to be "quality" bars. Everything else will be a bit of a compromise.

It's cheaper to buy a lighter barbell than it is to buy a sturdy but light bumper plate. So my advice is similar to the above: A 30 lb. barbell and 35 lb. bumpers will last a heck of a lot longer...
