Medical History Questionnaire

It seems that many affiliates have different ways of initial assessment of their clients. As far as paperwork it seems that many use the PAR-Q and other similar forms. I decided to take a version of the PAR-Q it and make up my own form (thanks Frank Dimeo for the hard copy version!).

I'm still new to the fitness industry so I'll take any comments you have on it.

If you want a 2 pager- here it is. I also sell supplements, so this questionaire helps me dial in their nutrition and possible supplementation use. Didn't give this to you before because I just finished it. Let me know what you think.

Prints out as 2 pages--might need to adjust your page setup before printing
anyway, so far everyone likes it. they fill it out and then we sit down and go over it. i am very glad that I have it in place now

Oops! that is not my doc. that is of course Andy Petranek's. I used it as a template to generate ideas. Sorry to everyone. here is mine, and it is only 2 pages long

Ha, ha! I thought it might be something like that.

It looks really good - I may have to run this one by my 'team' for approval. It is a little more thorough, but to the point as far as our CF standards/goals are.
