Hi guys,

Wondering if anyone has a health questionnaire they get their clients to fill in when they start? I have one but I'd like to get a better one, make sure I'm leaving nothing out.

In Canada we have something called a PAR-Q, a basic form provided by Health Canada to assess potential risks. If they "fail" that questionnaire, they are required to have a physical where a doctor fills out the Parmed-X form and returns it to you.

Basic google search will bring up both forms.
