I've found that ego gets the best of most, and regardless of misses, they retain the weight and try to "grind through" the movements.
I always sorta had a rule for myself. If I missed 2 or 3 times, I dropped weight. Even if I knew it wasn't a strength issue. I'd drop weight and rep my way back up reinforcing better form, as well as confidence in the lift.
