It's hard to know exactly what it is you're trying to describe, but the fact that you get the pain at the top of the lift when the shoulder joint is fully abducted and extended suggests some kind of shoulder impingement, probably of the supraspinatus muscle. Also, if you are having problems with the 'tracking' of your shoulder it suggests there might be some kind of rotator cuff weakness or imbalance.

What is your experience with exercise/lifting? Are you relatively un trained or have you been doing it for a while?
