If you are not a developer yourself you'll want to find one that very closely fits your desired look and feel. There are literally thousands out there so it's hard to suggest just one.

There are many contact form plugins available. Just look and find the one that fits your needs the best since it doesn't sound like you'll be able to customize it yourself.

You wouldn't be able to fit it into a sidebar, but you could embed just the scoring page like they do (link below - adjust parameters as needed). You'd need to put it in your main content area though...

I don't think you'll see anything that like unless it's released from CrossFit themselves. Reason being is that they do not provide an API, or "interface", for developers like me to access that data...

I looked at the site in your sig and while its not the most flashy looking site, its certainly not hard to navigate. What exactly are you having issues with?
