If you've been in that much pain for so long, the first thing I would do is stop for a week or two 'til it heals up, then TAKE SOME WEIGHT OFF THE BAR.

After that, i'd worry about low bar vs high...

Weightlifting shoes? They help keep the weight on your heels if you haven't tried them.

I do the same thing at maximal loads too, frankly. Last time I squatted, 4 of 5 reps during the heaviest set...

I can do exactly the same number of deadhangs and kips. It's been that way for a while, despite both numbers climbing gradually. Maybe my kip just needs more work.

Work in plenty of 'skills' days at the beginning. This will accomplish 2 goals:

1) increasing skill makes working out more fun and rewarding, both mentally and physically;
